Occupational Therapist Wanted for Skilled Nursing Facility
We are seeking a Full-Time Occupational Therapist to join our team at Avante at Ocala, a Skilled Nursing Facility. This is a vital role in a facility with a positive community reputation, a history of successful outcomes, and an experienced cohesive team.

As an Occupational Therapist, you will provide and direct Occupational Therapy services to residents, including assessment, treatment, program planning and implementation, related documentation, and communication. You will work under physicians' orders and be responsible for:

--- Providing Occupational Therapy services to residents and initiating referrals when appropriate and in accordance with department procedures.
--- Conducting screening of residents at regular intervals to determine need for intervention/treatment.
--- Evaluating residents to obtain data necessary for treatment planning and implementation.
--- Monitoring resident's response to intervention and modifying treatment as indicated to attain goals in accordance with physician orders.
--- Developing appropriate home or community programming to maintain and enhance the performance of the resident in their own environment.
--- Providing clinical information in a timely manner concerning safety and functional needs for discharge.
--- Reviewing the quality and appropriateness of the total services delivered and of individual occupational therapy programs for effectiveness and efficiency, using predetermined criteria.

**Requirements:**

--- Bachelor of Science in Occupational Therapy from an accredited program.
--- Successful completion of National Certification Examination for O.T.R.
--- Current state licensure or license eligible.
--- 0-3 years of long term care experience.
